Jim,
An idea would be to flash a strobe at the foreground subject during the exposure and then getting out of the frame. Then you don't have to sit motionless for the whole exposure.
It looks like Gemini with Saturn in the star trails.

Hi Jim, GREAT shot I really like...I too am from the Bay Area but am in Idaho now..=( Thank you for the comment on my sons xray... it was a shot at our chiropractors. He is working on my son and the shunt is a zipper. I miss the bay area beaches and GREEN so take lots of pictures send me some sand and beach smells. I will pop back to see your work again. Great idea Jim and so very well excecuted. Nice to meet you this way. Excellent exposure and beautiful movement in the stars due to long exposure time. How long did you have to sit still for this shot Jim....Best regards Harry

Judging by the trail of the stars, you probably had to sit still for a long time. Really appreciate your interest and passion for photography.....I mean I haven't seen 2.00 a.m in ages let alone be up and about toting a camera :-) regards Mathew.
