Q. bad quality results, and i need to know the cause .

i have bad quality results
(first) i have this mark inside of the white box, it appers on alot of pic, what might be the couse of it, is the problem from the lens, or the camera.
(second) on the edges of the pic and the corners you can see the alot of white pixels and noise, i know the pic is under-exposure, but this happen too with another pic, you can check it on this pic you will see it on the righ side (http://www.usefilm.com/image/970788.html)
camera: Canon eos rebel t2, 300x
lens: canon 28-90mm f/4-5.6
film: kodak gold 400.
pic: 1 sec, 16
hope that someone can tell me the cause of that.

Do you have another lense you could use, just to see if it IS the lense you were using? Switch lenses, shoot a roll, and see if its still there. If it isnt, then you'll know it was the lense.
If you dont have another, go to a used camera shop when you have only a few shots left on a roll. Ask to put one on, take a few of the guy who works there. (Camera shop guys are usually used to this kind of thing, i doubt that they would find it to be a problem...)
It kind of looks like a piece of fuzz...
Have you tried removing the lense and blowing some air in with a bulb blower?I wonder if you have some hair stuck in the shutter screen.

I purchased a scanner one time and the images looked like that and I took it back.Got an Epson and everything was aok.Did you scan this image on a flatbed scanner?

Since your using film, the camera is box with a timer.. It looks to me you have a large piece of dirt inside of the lens ... changing lens do you get this..no other lens find a friend with a canon lens and shot a roll..
Suggestion.. use a middle F-stop F8-11 on these type of pictures that reduces the star effect on the lights.. stopping down makes the stars bigger..
The grain(on film) noise(on digital) does it do this all the time, are all you images underexposed, meter is not working properly?

In addition to the question already asked about how it was scanned:
Is this a neg scan or a print scan?
For the first mark (which does look like it might be a hair, or similar, in the film gate)-
Is it absolutely identical every time it appears, or is it just similar?
Does it always appear in exactly the same place on every frame?
If you look at the negative do you see any marks where the mark is on the scan? On the print (if printed)?
For the second marks: they look like they could have been in the original scene. Is this a possibility?

thanks don and all for your help .
the problem that i really cant find any other canon lens !!! it is strange, but am in egypt, till then i just have my lens.
i can see some dust from the inside of the lens, but i don't know how exactly i can clean this.
and for the underexposed pix, yeah some times i took underexposed pix, like in this pic, i want it to be a little dark with this reddish colour. so how can i avoid this

this is a negative scan, and it always there at the same place with the same size, and i don't see it in the negative, i just feel silly now because i didnt this like that, it might be from the scan it self not the negative ??!!
and for the 2nd part, i doubt it in fact

Avoid the underexposure by metering properly... either increase the exposure or up the ISO till the meter is correct.. I'm sure you will need a tropod for this work... from other answer it looks like your dirt is in the Scanner??
