A simple but effective idea, with a unique message as well.
On a constructive note, I see that the black border is ragged from the photoshop work. A minor point but a little distracting. You could mask that more effectively without too much difficulty. There is enough difference in color and luminosity that the mask construction would be really easy. Even a manual mask would be easy.

there was a time I think it's all craps about, but, now, I spend at least three nights a week by Pills! and I'm just 24!
yes, you are right, sometimes it's the only way!
even for me who had no pills for more than 20 years!
