Aha. i see, so the 'lighten up' retouch tool brought out a selective graininess where you used it, that is what my eye caught. see how some areas are blurry or normal in pixelation? but around the pictureframe it gets very grainy and the color/contrast is sharpened? thats from your lighten up retouch and that is what caught my eye as odd.
Its a very nice image!

Dear Sam This was an avaible light underexposed shot. The sole PS effect was a 'lighten up' retouch brush on the frame of the old family portrait on the table. There was no blur effect, nor color effects...

I like the aged portrait in the background, right. what kind of effects are you doing? it looks odd. did you add spurting red like blood? blur effect to a few places? hmm.
